Job Overview
The Home & Health Care Service Coordinator position will function as an AltaMed PACE ambassador, is welcoming and attentive to the needs of all patients in continuity of care, and create a positive patient experience by managing expectations while demonstrating compassion and respect. The role ensures continuity of participant’s healthcare needs at the custodial and/or SNF institutions with scheduled & routine provider on site/ remote assessment/ reassessment. The role will serve as a central point of contact, coordinating services, and providing support to enhance participants' overall satisfaction and well-being at the facilities, and/or at home. The role is that of an integrated member of the PACE Home and Health Care Team, under the supervision of the Manager of PACE Nursing Care Coordination and Home Care, in areas around patient satisfaction and delivering continuity care.
Minimum Requirements
High school diploma required; some college education/credits in healthcare, preferred.
Minimum of 2 years of experience working with physically or mentally impaired older persons is strongly preferred.
Experience working with the elderly patient population.
Minimum of 1 year of experience in a health care setting that supports provider and/or patient engagement departments required.
Minimum of 1 year of experience in process improvement and/or other quality improvement-related experience is preferred.
Current BLS/First Aid Certificate required.
Bilingual: English/Spanish/Mandarin/Cantonese preferred.
Compensation
$24.00 - $28.80 hourlyCompensation Disclaimer

The range is subject to change.
