Position Title:Employment Specialist - Supported Employment

Location: Owensboro, KY

Description:

Job Summary:

Responsible for providing the day to day supports needed by the people receiving services to accomplish their stated goals, documenting the supports provided, attending to the medical, health, safety and training needs of the individuals receiving services, and providing transportation as arranged.

Essential Duties:

  • Develop appropriate job opportunities for selected candidates, provide job coaching and supervision to people receiving services, arranges for follow-up, retraining, or ongoing support for people receiving services.
  • Complete documentation using correct terminology on the daily/weekly notes, and data sheets. Submit all paperwork by required deadlines.
  • Prepare and complete job training evaluations for individuals receiving services. Reviews evaluation with individual and develops a training program
  • Ensure safe delivery of services by maintaining a safe working environment and reports any unsafe conditions or acts to the supervisor and documents on an incident report. Participate in mandatory safety training required per regulations
  • Act as a liaison between participant, family, support coordination agency, and employers. Maintain regular communication with the support coordination agency to ensure effective communication regarding consumer’s needs. Participate in the Self-Assessment process / ISP meetings with the assigned Support Coordinator.
  • Complete initial and annual training as directed by agency policy and regulatory standards. Attend monthly staff meetings, and additional staff development training as scheduled. Monitored through annual staff development records and monthly in-service records by supervisor

  • Adheres to agency policies and procedures.
  • Treats all consumers with dignity and respect, and supports consumers to exercise their rights on a daily basis.
  • Cooperate and work as a team member with supervisor and follow employees and maintain a positive attitude.
  • Other duties as assigned.

Qualifications:

Education and Experience:

  • High school education or GED preferred with a minimum of three (3) years’ experience working with individuals with disabilities; or
  • Associates degree in a Human Service field and preferred 1 year experience working with people with disabilities; or
  • Bachelor’s degree in Human Services.
  • Valid Driver’s license.
  • Suitability for employment is subject to a criminal background check.
