We are a dynamic team that provides Care Coordination services to members enrolled in the Comprehensive Community Services (CCS) program here in the greater Milwaukee area. JFS CCS provides rehabilitative and recovery services to individuals struggling with mental health or substance abuse issues within the context of their unique needs and abilities. The programs offered through CCS are designed to promote and support recovery by stabilizing and addressing an individual's critical mental health needs, including substance use problems; an individual's ability to self-manage their physical and social health; and an individual's ability to meet their basic needs, including housing, education, and employment skills.

We are looking for someone with a 4-year Bachelor's or a Master's degree in a Human Services field (Social Welfare, Psychology, Social Work, etc.) AND have at least one-year of paid, formal (non-family care giving setting) experience providing case management services and working with with behavioral health, mental health and/or addiction issues. If you have the ability to work independently, stay organized, are detailed-oriented, able to meet deadlines (enter timely case notes, submit reports, etc.), and most importantly, conduct assessments of, monitor and support CCS consumers with a professional and caring methodology, we would like to meet you.


Our Care Coordinators meet with individuals in their homes or out in the community to identify recovery goals, develop customized recovery plans focusing on individuals' preferences in order to improve or maintain the members; quality of life, independence, health and overall wellness as it relates to their mental health or substance recovery. As a Care Coordinator, you and the consumer will work with the consumer and a dedicated team of providers to decide which programs and supports will assist the individual reaching their recovery goals.


The Care Coordinator position is full-time, predominantly first shift position requiring a valid Wisconsin Driver's License, access to a vehicle, personal motor vehicle insurance meeting the minimum requirements of the Agency, passing of a Department of Justice Background Check and Federal Sex Offender Registry Check, and verification of references.



General

  • Must have a valid driver's license, access to reliable vehicle, and automobile insurance coverage throughout employment and successfully pass a MVR and insurability check.
  • Knowledge, understanding, and/or willingness to learn Jewish culture and traditions and those of the Agency population as necessary.
  • Accept other duties and responsibilities as assigned.
  • Candidates must successfully pass a criminal background check with the Wisconsin Department of Justice and a caregiver background check with the Wisconsin Department of Health Services, the Department of Children and Families and the Department Safety and Professional Services.
  • Candidates must pass a Dru Sjodin National Sex Offender and Wisconsin Sex Offender Registry screening.
  • Vaccinations per program requirements (Flu, MMR, Varicella, TB Skin Test)
  • This job is an in office position with hours required to support the CCS team.

Qualifications/Requirements:

  • Four-year Bachelor's degree in a Human Services field (e.g. Social Welfare, Psychology, etc.) is required.
  • A minimum of one (1) year of paid formal (non-family setting) experience working with adults providing Case Management or related services is required.
  • Experience in behavioral health, mental health and/or addiction is preferred.
  • Experience in various case management systems (TCM, CSP, Wraparound Milwaukee, My Choice Family Care, etc.) is preferred, but not required.
  • Must have excellent interpersonal, organizational, critical thinking, problem solving, creative and independent thinking skills.
  • Must be able to work independently with minimal supervision.
  • Excellent verbal and written communication skills are required.
  • Excellent customer services skills are required.
  • Have the ability to make accurate assessments, interact effectively with consumers, agency staff, and other community professionals within a team concept.
  • Must be able to handle multiple tasks simultaneously and prioritize based on severity of need.
  • The ability to respect and maintain privacy and dignity of consumers and assure consumer confidentiality at all times is required.
  • Familiarity with Electronic Health Record (EHR) management systems is required.
  • Proficient in computer use, Microsoft Office applications
