Overview

This advanced practice provider position provides care to patients primarily in the outpatient setting but may include inpatient care and consultation as well. The clinic generally conducts patient care visits 2-3 days per week with additional inpatient care, procedural care, and research and clinical care coordination activities through the week. The Cardiology service provides age appropriate care to age groups indicated.

Scope of range within unit population: Adolescent through geriatric cardiology patients.

FUNCTIONS/DUTIES

Standard 1: Develops, interprets, implements and documents a plan of care for each patient and safely delivers this care.
A. Application of Area-Specific Process Model
a. Functions primarily within the Outpatient Clinics Practice Model
B. Clinical Process
a. Clinical Skills and Knowledge
i. Elicits patient history pertinent to current disease status and treatment
ii. Performs physical examination
iii. Analyzes patient information and laboratory data to form differential diagnoses
iv. Formulates plan of care obtaining any additional lab tests or studies, arranging return visits, providing appropriate patient education or referrals/consults for newly identified problems
v. Arranges support services as needed in conjunction with discharge services and social work departments.
vi. Participates in care conferences and discharge planning conferences
vii. Collects, prepares and submits data for review in accordance with collaborative study groups and international data registries.
b. Emergent Situations
i. Contacts Cardiology attending or fellow to discuss emergent situations and plans
ii. Recognizes unusual or untoward reactions to interventions, treatments or related to their condition and contacts attending physician
c. Patient Education
i. Provides education for patients about their disease, treatment and potential complications
ii. Discusses risks/benefits when prescribing medications or treatments/ interventions.
d. Patient Rights
i. Provides patient care based on the "Patient Bill of Rights"
ii. Respects patient diversity in beliefs and health care concerns
iii. Assists patient in problem solving health care concerns
e. Documentation of Practice
i. Records all patient visits including lab results, medication changes, current plan of care, return visits and new problems in the inpatient or outpatient charts.
ii. All documentation is completed timely and in accordance with OHSU policies and Meaningful Use objectives.
C. Coordination of Activities
a. Time Management
i. Available for scheduled and unscheduled, urgent outpatient clinic visits or inpatient visits
ii. Provides patient follow-up in a timely manner
iii. Requests RN and clerical assistance to meet patient follow-up needs
b. Communication/Collaboration
i. Demonstrates sensitivity in patient and family interactions
ii. Respects cultural and ideological concerns of patients and families
iii. Interacts with other care providers on a collegial level
iv. Collaborates with other health care providers as needed to provide coordination of patient care
v. Arranges channels of communication for patient problems needing urgent or emergent follow-up
D. Continuous Quality Improvement
a. Risk Management
i. Reviews urgent and emergent problems with Cardiology fellow or attending
ii. Adapts clinic follow-up guidelines to meet individual patient needs
iii. Uses universal precautions with all patient contacts
b. Area-Specific Quality Improvement
i. Identifies desired patient outcomes
ii. Gathers data reflecting achievement of desired outcomes
iii. Adapts patient care based on continuous quality improvement information


Standard 2: Participates in education programs and research that contribute to the improvement of advanced practice.
A. Development of Professional Knowledge
a. Assumes responsibility for own professional development
i. Participates in pertinent conferences and educational meetings
ii. Obtains educational hours as needed for state and national certification
iii. Completes all university required education program activities within established guidelines
b. Bases practice on current research and standard of practice
i. Maintains currency within the specialty of Cardiology
ii. Maintains currency within the primary care of adult patients
B. Education of Others
a. Participates in the development of patient education tools and programs
b. Provides individualized patient education on aspects of cardiology treatment
c. Assumes responsibility for educating others on aspects of cardiology and cardiovascular treatment, caregiver support and guidance
i. Provides student lectures and learning materials, as requested
d. Participates in the teaching of medical students, residents, fellows, clinical research staff and the regional community

c. Participates in development of practice standards involving cardiology nursing and therapy for cardiovascular disease.
d. Participates in developing clinic forms and patient education to meet the needs of the outpatient service
E. Research
a. Functions as co-investigator on clinical research studies
i. Consents research subjects for study participation
ii. Interviews and screens patients for entry onto protocols.
iii. Monitors and documents response to therapy and communicates findings to principal investigators with recommendations for appropriate intervention.
iv. Participate in clinical research meetings with OHSU team, with study sponsors, and with others involved in the research conduct
v. Collaborate closely with study coordination team to ensure safe, efficient and compliant implementation of study protocols, and to ensure integrity of research data
vi. Ensures accurate and timely reporting as required by regulations, OHSU policy or study sponsor

A. Resource Utilization
a. Uses basic support systems (human, material, and technology) to reduce care costs and improve efficiency

Responsibilities

Nurse Practitioner:

  • Current Nurse Practitioner license for the state of Oregon
  • State of Oregon Nurse Practitioner Prescriptive Privileges
  • ACNP or AGACNP required for inpatient services
  • ACLS and BLS certification

Physician Assistant:

  • Current & Active Physician Assistant license for State of Oregon
  • DEA certificate
  • ACLS and BLS certification

Note: A completed practice agreement is required, post-offer/pre-hire.


Experience:

  • 2 years prior experience as an APP
  • Experience working with cardiology patients
  • Experience working with patients/participants in clinical research/trials is preferred
  • Experience in an academic medical center
  • Membership in a professional organization
